Informed Consent, Confidentiality and Liability Statement
for Therapy Sessions with Chelsea Rose
When participating in a therapy session, it is important that we have a shared understanding of our mutual responsibilities and the conditions we agree upon. Therapy sessions provide a space and opportunity for you to explore behaviour, relationships, feelings or thoughts which trouble you and cause difficulty for you. This is also a legitimate source of support during a difficult time. Therapy can bring deeper personal insight and awareness, better ways of understanding and coping with problems, and improved relationships.
Although there can be many benefits of participating in sessions, I acknowledge that it can also be a difficult process that may lead to the opening up of emotional pain. Therapy sometimes invites you to be willing to examine difficult topics or events in your life, to experience stronger than usual emotions, and to try out new and different behaviours. This may disrupt your relationships and daily activities. As I support you in your therapy journey, I will endeavour to create a safe and attuned space for challenging material to be processed.
If you ever have questions or concerns about any aspect of our work, please do ask me. Our sessions are booked in advance and I do not offer immediate crisis response. In the case of a crisis, please contact your doctor, an emergency helpline within your country, or go to the nearest hospital.
Personal information gathered in the course of therapy will be used in accordance with the purposes outlined in the paragraph above and will not be disclosed except in cases of the exceptions outlined below. Confidentiality is a key to the effectiveness of the therapy process, so the personal information you share in sessions will be kept confidential. Confidentiality continues after the end of the therapeutic relationship. There are, however, some exceptions to the therapist’s duty of confidentiality, in particular:
(a) If it is my opinion that a child is or may be at risk of abuse or neglect, or in need of protection;
(b) If it is my opinion that you or another person is at clear risk of imminent harm;
(c) If I am called to speak in legal matters where keeping confidentiality would be in contravention of the law.
I occasionally discuss my work with other therapists. While I may disclose information for the purpose of a professional consultation, your identity will remain confidential. In therapy, it is your right at any time to:
(a) have a review of your progress and of any of the topics in this form;
(b) be provided with a referral to another therapist or health professional;
(c) withdraw consent for the collection, use, or disclosure of your personal information, except where precluded by law;
(d) end the therapy relationship by so advising the therapist;
(e) access or obtain a copy of the information in your counselling records, subject to legal requirements.
This agreement is governed by the laws and regulations of the resident country of the therapist fulfilling the work and any dispute shall be resolved in the therapists’s resident country.
By commencing somatic psychotherapy sessions with Chelsea Rose you are in agreement to the above terms.
